What Is the Malaysia Health Declaration Form?

The Malaysia Health Declaration Form (commonly completed as part of the Malaysia Digital Arrival Card — MDAC) is an electronic pre-arrival health self-declaration required by Malaysian authorities for most incoming foreign travellers. The system was rolled out alongside MDAC in late 2023 and became mandatory for many visitors in early 2024, replacing earlier paper arrival health questionnaires and ad-hoc checks at immigration counters. The health declaration collects basic health screening questions, recent travel history, and contact/accommodation details to support entry checks and public-health monitoring.

You submit the declaration through the official MDAC/MySejahtera channels (the government portal and MySejahtera Traveller page) or use DoVisa for guided completion. The online process asks for passport details, flight and arrival information, accommodation address, and simple yes/no health questions. After submission you typically receive an email confirmation and a reference/QR code to present to your airline at check-in or to immigration staff on arrival — keep this confirmation accessible offline.

Although the Malaysia Health Declaration Form helps streamline arrival screening, it is not a visa or entry permit and does not replace any visa or passport requirements. Immigration and visa decisions remain the responsibility of Malaysian Immigration; the declaration only records health-related information for arrivals. Key policy updates: MDAC and its health declaration elements were introduced across late 2023 and made broadly mandatory from 1 January 2024 — always verify the latest timelines on the official MDAC page.

For full entry rules see Malaysia visa information and the government portal at IMI MDAC announcement. When you're ready, Apply for your Malaysia Health Declaration Form now with DoVisa for guided help and confirmation checks.

  • Airline and vessel crew members on active duty who use crew channels
  • Transit passengers who remain in the airside transit area and do not clear immigration

Malaysia Entry Requirements & Restrictions

Passport Validity

Your passport must be a valid recognised travel document. Malaysian Immigration guidance recommends at least 6 months validity from the date of arrival; check your embassy guidance before travel. See Entry requirements — IMI.

Vaccination & Yellow Fever

A Yellow Fever vaccination certificate is required only if you are arriving from a country with yellow fever transmission risk. No routine COVID-19 vaccination proof is required for entry as of current guidance. See CDC Malaysia for health recommendations: CDC — Malaysia.

Customs & Prohibited Imports

Malaysia maintains strict controls on prohibited and restricted items. Recent changes include an absolute ban on e-waste imports effective 4 February 2026. Declare controlled items and check Malaysia Customs — Prohibition of Import & Export for full lists.

Health Screening & Documentation

Complete the online health declaration before arrival through MDAC/MySejahtera. Airlines and immigration may request your confirmation QR/reference at check-in or on arrival. Keep vaccination certificates ready if required by travel history.

Travel Insurance Recommendation

While not mandatory for the health declaration, comprehensive travel and medical insurance is strongly recommended — medical evacuation and treatment may be costly in Malaysia's major cities and remote areas.

Travel Tips for Malaysia Visitors

Malaysia is a Southeast Asian federation split between Peninsular Malaysia and East Malaysia on Borneo. Major international gateways include Kuala Lumpur International Airport (KLIA), Penang International Airport (PEN), and Kota Kinabalu International Airport (BKI); major seaport gateways include Port Klang. Domestic transfers and regional connections are well developed.

  • Currency: Malaysian Ringgit (MYR). Major tourist outlets sometimes accept USD, but carry MYR for taxis, markets and smaller vendors.
  • Language: Malay (Bahasa Malaysia) is the official language; English is widely understood in cities, airports and tourist areas.
  • Time zone: Malaysia Standard Time is GMT+8 with no daylight saving.
  • Best time to visit: December–March for drier weather on the east coast of Peninsular Malaysia and Sabah; expect heavier rains in other months in some regions.
  • Airport transfers: KLIA is the main international hub — allow extra time for immigration and baggage if you have a short domestic connection.
  • Customs: recent import rule changes (including an e-waste import ban) mean you should check prohibited items before travel and declare any restricted goods at arrival.
  • Local laws: Be mindful of public conduct and respect local religious norms — dress modestly in rural and religious sites.
  • Save your confirmation: Screenshot or download your MDAC/health declaration confirmation (QR or reference) — airlines commonly check it at departure and immigration scans it on arrival.
