Malaysia Health Declaration Form

The Malaysia Health Declaration Form (commonly completed as part of the Malaysia Digital Arrival Card — MDAC) is an electronic pre-arrival health self-declaration required by Malaysian authorities for most incoming foreign travellers. The system was rolled out alongside MDAC in late 2023 and became mandatory for many visitors in early 2024, replacing earlier paper arrival health questionnaires and ad-hoc checks at immigration counters. The health declaration collects basic health screening questions, recent travel history, and contact/accommodation details to support entry checks and public-health monitoring.
