Mandatory Health Declaration form before travel
All travelers must complete the online Self Declaration Form before travel to Malawi. Travelers can complete the form up to 2 days before departure to Malawi. Once the form is completed, the traveler will receive a confirmation with a QR Code. The QR Code can be saved to the traveler's mobile device or printed and presented to Immigration or Health officials upon arrival.
