The form is completed and signed by one parent or guardian, and requires no administrative validation. The minor must carry the signed original, a legible copy of the signing parent's identity document, and their own valid passport or ID card (depending on the destination requirements). The requirement applies whenever such a minor travels without a parent or guardian, regardless of destination. Always check with your airline, as they may have additional requirements for minors beyond standard rules.
