Guam Guam Health Declaration Form
Complete the Guam Guam Health Declaration Form online — mandatory electronic health and customs declaration available up to 72 hours before arrival.
What Is the Guam Guam Health Declaration Form?
The Guam Guam Health Declaration Form (also published as the Guam Electronic Declaration Form or EDF) is the mandatory digital health and customs declaration all arriving passengers must complete before clearing immigration in Guam. Managed by the Guam Customs & Quarantine Agency (CQA) and implemented as an electronic-only submission from February 4, 2025, the EDF replaced legacy paper arrival cards and enables health screening, customs declaration and arrival triage in one online step.
You submit the declaration through the official Guam EDF portal (guamedf.landing.cards) or via kiosks at the port of entry if you are unable to complete it pre-departure. On approval you receive an email confirmation and a QR code/confirmation that airlines and immigration officers scan at check-in and on arrival; keep the QR code offline for fast access at Antonio B. Won Pat International Airport (GUM).
The Guam Guam Health Declaration Form is strictly a pre-arrival health & customs declaration — it is not a visa or travel authorisation. It collects contact, travel-history and health symptom information and helps CQA and federal partners identify public-health risks and customs issues before passengers enter Guam. Travelers should still meet U.S. entry rules where applicable (ESTA/visa requirements) and carry travel documents as required.
For full entry rules see Guam visa and entry information and the official CQA portal (cqa.guam.gov). Ready to apply? Apply now to complete your declaration with guided help.

Guam Entry Requirements & Restrictions
Passport & Entry Documents
You must carry a valid travel document — your passport should be valid for the duration of your stay. Visa or ESTA requirements for entry to the United States also apply to Guam; check U.S. entry rules and your nationality-specific obligations on the official sites.
Vaccination & Health Requirements
No routine vaccination is universally required for entry to Guam; however, a Yellow Fever certificate is required if you are arriving from or have transited through a country with yellow fever risk. See CDC guidance for details: CDC — Guam.
Customs: Prohibited & Restricted Items
Guam prohibits or restricts import of meat and meat products, plants and plant products, weapons and ammunition, explosives, protected wildlife and other regulated goods — such items may be seized and penalties applied. Review Guam Customs & Quarantine Agency import rules: CQA — Import Requirements.
Health Screening Information
The electronic declaration gathers health-screening data (symptoms, travel history). Travellers with concerning symptoms may be referred for assessment; follow instructions from CQA and local health authorities at arrival.
Travel Insurance & Medical Care
Medical facilities on the island are limited for complex evacuations — travel insurance with medical evacuation is strongly recommended though not mandatory.
